Arizona Tax Form 5000 - Alumi-Cover requires Transaction Privilege Tax Exemption Certificate unless you wish to pay sales tax on your purchases. Download the form.

New Mexico Tax Form - Alumi-Cover requires you to have a Non-Taxable Transaction Certificate for your company unless you wish to pay sales tax on your purchases. This form is availavble from the NM Taxation and Revenue Department office nearest your location.
